Decoda Raises $4.5M to build the Operating System for Modern Care

We've raised $4.5M from Y Combinator, The Fintech Fund, Graphene Ventures, and several operators across healthcare and AI to build the coordination layer that modern clinical care runs on. Today, Decoda is already live in hundreds of clinics across the United States, processing approximately $100M in annual payments and engaging thousands of patients every day..

This work is anchored in a simple idea: healthcare works when continuity works. Continuity works when memory works. And memory breaks in today's clinical systems. We're here to fix that.

The Memory Problem

Clinics rarely struggle because providers lack skill or because patients don't want care. They struggle because the operational layer of healthcare does not scale. Phone calls go unanswered, messages accumulate, documentation is rushed, follow-up plans weaken, and patients quietly fall out of the care pathway. These gaps aren't caused by neglect, but by the limits of systems that rely on fragmented information scattered across inboxes, EHR fields, manual notes, and human recollection. When memory is inconsistent, continuity becomes optional—and continuity is what drives outcomes.

How Decoda Works

Decoda replaces that fragility with a reliable memory and coordination layer. It answers calls and messages instantly, books visits with patient context, summarizes encounters and writes clinical documentation automatically, maintains treatment follow-ups and reactivations, assigns codes, submits and tracks claims, and manages the entire billing cycle. It does all of this without adding staff, forcing workflow changes, or requiring clinics to learn a new set of tools. Decoda simply makes the clinic function as if it were one connected system.

Why This Matters

This matters because clinical care is shifting. Patients want ongoing guidance, not episodic appointments. Providers want to spend time practicing medicine rather than coordinating logistics. Organizations want to scale without multiplying administrative overhead. The limiting factor is not demand, medical knowledge, or reimbursement—it is the ability to maintain continuity reliably over time. Decoda enables clinics to remember and act consistently, across staff transitions, patient histories, treatment phases, and growth. When patients feel remembered, they stay engaged; when they stay engaged, outcomes improve; and when outcomes improve, clinics are able to scale effectively.

Our Vision

Our goal is not to automate tasks, but to make clinical operations self-coordinating. A clinic should feel like one continuous system, not a collection of disconnected tools. Administrators should be able to focus on care and organizational performance rather than tracking loose threads. Providers should operate at the top of license. Patients should feel understood, not managed. Decoda is the operating system that makes this possible.

We are now hiring the first ten engineers and product builders to help define this architecture. The work focuses on distributed state across care environments, real-time reasoning loops, clinical event-driven systems, and scalable human-in-the-loop reliability. Healthcare is becoming more continuous, more personalized, and more human. We're building the system it will run on.
