Decoda Health Raises $4.5M to Build the Operating System for Modern Care

Healthcare has advanced clinically, but the software powering clinics hasn't kept up. Outdated Electronic Medical Records (EMR) force teams into manual work, fragmentation, and burnout. Clinics need technology that thinks with them, not tools built decades ago.

Today we are excited to share that Decoda Health has raised $4.5M to power our next stage of growth. This includes a $3M seed led by Y Combinator, with participation from The Fintech Fund, Graphene Ventures, and operators across healthcare. The round builds on a previously unannounced $1.5M pre-seed, and accelerates our mission to build the first AI native PMS and unified operating system for modern clinics.

Why Now

AI is transforming how work gets done. In healthcare, where workflows are dense and interconnected, the gap between AI-powered clinics and those relying on manual operations will widen quickly. Practices can't sustainably hire more administrative staff for every additional patient, they need intelligent systems that automate and coordinate work.

Administrative costs account for up to 25% of U.S. healthcare spending and physicians spend nearly 6 hours a day in legacy PMSs. AI for complex workflows is only now ready for healthcare, while clinics cannot keep hiring more administrative staff as they grow. The timing could not be better for a platform built from the ground up with intelligent automation.

Our Solution

Decoda is an AI native Operating System (OS) built around intelligent agents rather than legacy workflows. We started with two core products:

  • AI Receptionist that answers calls, books appointments, and updates records automatically
  • AI Scribe that generates structured clinical notes and extracts key details like procedures and follow ups

From there we built the full set of modules every clinic needs. Decoda includes scheduling, intake and consent forms, a unified communication suite, charting and documentation, inventory and checkout, patient messaging, and reporting and analytics.

The result is a platform that goes far beyond a simple LLM wrapper. Decoda is already delivering meaningful impact at scale:

  • $20M revenue captured for customers using AI
  • Over 5M patient interactions handled
  • $100M dollars in payments processed annually
  • Live in 150 clinics across the US

Because these agents and modules live inside the same platform, every workflow becomes connected and actionable.

Examples:

  • If a patient calls about a service but doesn't book, Decoda tags them so clinics can follow up.
  • If the AI Scribe detects a procedure, the system auto-populates checkout, inventory changes, and future reminders.

This makes Decoda not just any regular PMS - but the operating system for modern clinics.

We started in elective care - medspas and weight-loss clinics - so we could master clinical operations without the drag of insurance complexity. Now, as we roll out our insurance-billing infrastructure, we’re expanding into cosmetic surgery, dermatology, optometry, behavioral health, physical therapy, and dentistry.

Our goal: when clinicians think "I need AI in my practice", they think Decoda.
